Self Evaluation Paper

The final project for this course is a three to five page paper (double-spaced).

Please note the due date listed in the Calendar of Assignments.

The paper includes a self-evaluation section, as well as a plan of action section. In this self-evaluation paper you will reflect on your own progress, as well as your plans for maintaining new behaviors you have adopted or maintained.

Format

The format of your paper should be written in paragraph form. However, use 'Self-Evaluation' and 'Plan of Action' as headers, and each numbered point as sub-headers, as indicated here:

Self-Evaluation (1-2 pages)

  • Current stage of a health behavior you focused on for lifetime fitness, and how it's changed over the course (3-pts)
  • What you've learned about yourself over the course (3-pts)
  • Positive changes you've made over the course (3-pts)

Plan of Action (2-3 pages)

  • Discuss how you plan to maintain a health behavior as it relates to lifetime fitness after this course. Try to be very specific about how you will maintain the behavior (e.g. weight training, aerobic activities, flexibility relation, dietary changes, how often, where you will do them). (3-pts)
  • Develop a one-month overview calendar to show your intended plans with the start date. (5-pts)
  • Develop 1 short-term and 1 long-term goal as it relates to your lifetime fitness using the S.M.A.R.T. guidelines. Also include 3 goal objectives for your short-term goal. (5-pts)
  • Discuss a barriers that you may need to overcome for each goal objective and at least one solutions for each barrier. (3-pts)
  • Discuss types of support and rewards you can use to help you reach your health and fitness goal(s). Part 5 will be for part of class participation in Lab 9.

Note: Points are associated with each section of the paper in order to give them full and proper attention.

This paper is worth 25 points and is due during the last week.

Attention: Please try to avoid the following misunderstanding. In Lab 9, there is a form that needs to be completed and turned in with your paper. This form is also entitled Plan of Action, but it is different from the component of your paper with the same title described above.

Instructions for delivering the final project assignment

  1. Develop your paper using a word processing program (Microsoft Word 2000 preferred).
    • If you use a Macintosh or Word Perfect, please save your file as .rtf, .txt, or .pdf.
    • Save the file to your hard drive.
  2. Attach or paste your work using the view/complete link found in the FINAL PAPER area.
  3. My comments on your work will show up in the gradebook.
  4. Please follow the due date listed in the Calendar of Assignments.
